Aged Care in your home - Central Coast

Aged care in your home is a great way to help you stay living safely and independently. You can start with a little, and if and when you need to, we can help with a bit more.

Access to subsidised in home support for all Australians over 65 (over 50 ATSI) to stay living independently at home is via the Federal Government's My Aged Care (MAC).

My Aged Care provides a gateway for older people, their families and carers to get information about and gain access to subsidised in home care services.

Once you have received a referral through My Aged Care, you can choose Adssi to deliver your in-home support. 

The Federal Government subsidises two programs:

Home Care Packages

At Adssi, everyone's Home Care Package (HCP) Plan is uniquely tailored to meet their specific needs and wishes. We welcome the input of loved ones and are very experienced at helping our clients identify and prioritise the best use of their Home Care Assistance Package. In-home aged care choices are extensive.

Some options to think about are:

  • Tasks around the house and garden, help with personal care - showering, grooming;
  • Help keeping you organised - making appointments, paying bills;
  • Getting back to hobbies and interests and being social;
  • Gentle exercise;
  • Equipment and technology to make activities easier;
  • Shopping and meal preparation;
  • Transport to appointments;
  • Home modifications (ramps & rails) to make things safer.

Home Care Packages (HCP) are available for frail older people who have been assessed by the Aged Care Assessment Team (ACAT).

HCPs range from Level 1 - 4 and provide low to high care needs from a few hours a week (Level 1), up to around 12 hours a week (Level 4). You can refer yourself for an ACAT assessment, or a friend or family member or GP can do it on your behalf.

Commonwealth Home Support Program

The Commonwealth Home Support Program (CHSP) provides subsidised in-home aged care services for people needing a little help. CHSP is designed to 'do with' not 'do for' you, in order for you to retain as much independence as possible. You can have help with services such as: Services include:

Household Help

Assistance with essential cleaning of areas in the home that are regularly used including the kitchen, bathroom and bedroom. The service can also include dish washing, laundry, ironing and meal preparation.

Linen Service

Providing clean towels, sheets, pillowcases and bed pads. Pick up and delivery is provided twice a week. We may also be able to help you with changing the bed linen.

Social Support

Enjoy the company of others on our supported outings and bus trips. Trips include visits to nurseries, the movies, lunches, variety concerts and shopping. Special events are held to celebrate things like Easter and Christmas.

Our Men’s Group meets fortnightly and provides time and a place for men to enjoy the company of other men. Guest speakers are invited to speak on interesting topics and lunch is provided. Excursions are also arranged to visit museums and places of interest.

Adssi is a proud sponsor of the Bateau Bay Men’s Shed where men are welcome to spend time working on projects for themselves or the community, share stories and a cuppa, learn or teach in a relaxed friendly atmosphere. You do not need a referral to attend the Men’s Shed – simply phone them on 0435 807 633.

Personal Care 

Offers in-home support with showering, toileting, dressing and grooming. We have highly trained male and female staff who treat you with dignity, respect and consideration.

You may require personal care assistance for a short period of time, following some time in hospital or recuperating from illness. Personal care can also be provided as a regular and ongoing service. We also have a team of qualified hairdressers (cuts only).

Flexible Respite Services

If you are a carer who provides significant levels of care and support to someone who has a disability, a chronic health condition or who is frail and aged, you may be eligible for in-home support respite services.

In-home respite gives carers a break from their caring responsibilities by temporarily taking over the caring role. The break could be for a few hours, a full day or longer – just what you may need to attend to your own health and personal needs, or simply to recharge your batteries.

Our partnership with Breakaway in San Remo enables us to provide getaways for carers of the frail aged and people living with dementia and their carers. Respite getaways at include all meals, activities and personal assistance all set in beautiful, natural surroundings.

Home Modifications and Maintenance Service

Our qualified building services team can help improve safety and access in and around your home with:

  • Installation of ramps, rails and other modifications that help you access and live in your home more safely
  • Handyperson services for small home maintenance and repairs such as minor carpentry, electrical and plumbing
  • Garden and lawn maintenance, one-off yard and rubbish clearance and gutter cleaning
  • Major bathroom modifications and the installation of lifts are also possible through an assessment process

Occupational Therapy

Our qualified, experienced and registered Occupational Therapists provide in-home assessments with the aim of improving your safety and independence both at home and in the community.

We will discuss what difficulties you or the person you care for may be having with simple day to day activities. It may be that you are starting to have difficulty getting out of bed or out of a chair, or that you are worried about having a fall.

The home assessment is comprehensive and will take into consideration any medical conditions, current medication, and your ability to safely access the home and community. We will identify risks and hazards that may compromise your safety or independence.

The Occupational Therapist will make their recommendations in consultation with you and/or your carer and will first identify any simple solutions. This might be assistive equipment available for purchase or hire or a simple grab rail modification in the bathroom or toilet. More complex recommendations may be required such as a full bathroom modification.

Physiotherapy

Our qualified, experienced and registered physiotherapists can come to your home to support you to get better and stay well.

Physiotherapists assess, diagnose, treat and prevent a wide range of health conditions and movement disorders. Physiotherapy helps repair damage, reduce stiffness and pain, increase mobility and improve quality of life.

Gardens and Lawns

We can help you with your garden and lawn mowing needs to help keep things tidy and safe.

ComPacks Service in Central Coast

After a stay in hospital people often need extra help for a short time when they get home. The ComPacks service is initiated by social workers or discharge planners while you are still in hospital.

Our qualified, experienced and accredited case managers provide assessment and case management services to assist a safe and supported return home from hospital for up to six weeks after hospital discharge. They can put in place short-term services such as domestic assistance, personal care, meal preparation and transport to and from appointments.

If services are required for a longer period the case manager can transition you from ComPacks into a program that provides the supports you require, subject to availability.

Please discuss this service with the hospital social worker, either before or during your hospital stay. ComPacks is a NSW Health initiative.
